What Is a Medicare Care Plan?
A Medicare Care Plan is a plan prepared by your GP to help you manage a chronic or ongoing medical condition.
If your condition has lasted, or is expected to last, for six months or longer, your GP may determine that you're eligible for a Care Plan. Examples of chronic conditions include:
• Back and neck pain
• Arthritis
• Sports injuries with ongoing symptoms

• Chronic shoulder or knee pain
• Stroke recovery
• Balance or mobility problems

How Does a Medicare Care Plan Work?
The process is simple:
Step 1: Visit Your GP
Book an appointment with your regular GP and discuss your ongoing health condition. If you're eligible, your GP can prepare a Medicare Care Plan and provide a referral to a physiotherapist.
Step 2: Bring Your Referral
Once you have your referral, simply contact Herbs & Motion and book your physiotherapy appointment.
Step 3: Begin Your Treatment
Our experienced physiotherapists will assess your condition, develop a personalised treatment plan, and work with you to improve your movement, reduce pain, and help you achieve your health goals.
How Many Physiotherapy Visits Does Medicare Cover?
Medicare may provide rebates for up to five eligible allied health services per calendar year. These visits are shared across all allied health providers, such as physiotherapists, podiatrists, dietitians, exercise physiologists, and others.
Your GP can explain how many visits are available based on your individual Care Plan.
